The ultimate cause of this disaster was a bearing that was installed backwards resulting in reduced cooling oil flow which caused the explosion in the gearbox.
to compound the issue in those days they would steam with the engine room hatch open meaning that the explosion spread outside of the engine room and down the flats and trapping several people in the main cafeteria.
A lot of improvements were made as a result of this horrific accident
