NORAD cannot launch without the President's permission. It's one thing to have something written down in a manual or policy paper. It's another thing to give the order that will wipe out millions instantly and affect your allies in the region. I don't see Barack Nobel-peace-prize Obama having the balls quite frankly. The U.S. using nukes, even tactical ones, would be a disproportional response (considering you can bomb N Korea further back into the stone age with about 48 hours of bombing sorties) and would only serve to anger allies and neutrals, encourage terrorism and more importantly (even though it shouldn't but let's be realistic) would be political suicide.
